*Description/Job Summary*The Medical Case Manager confers with doctors and RHD Salt Passaic Team to gain a complete grasp of the participant’s needs. The medical case manager assists in procuring all resources needed to serve the participant’s needs. The medical case manager consistently monitors and reviews the services of each resource, ensuring that progress is made in an effective and timely manner.*Responsibilities/Duties*1. Assist participant and RHD Salt Passaic Team with the development, implementation, and updates of participant’s Recovery Support Plans.2. Assist participants with obtaining and maintaining medical insurance coverage and problem solving medical coverage concerns, i.e., unresolved payments, prior authorizations, medical transportation, etc.3. Assist participants with identifying primary care physician, optometrist, dentist and medical specialist.4. Treat basic medical needs and dispense medications/treatments, as needed.5. Provide educational counseling and support for participants regarding their health care and recovery goals.6. Engage participants through face-to-face and over-the-phone interactions to assess overall medical health status and respond accordingly to any change in status. Document all interactions with participants.7. Track and document, participant’s medical needs, appointments, and trends.8. Act as a liaison with health care community organizations and resources, while empowering participants to be their own liaison and advocate.9. Provide on-going education to participants and RHD Salt Passaic Team regarding medication management and administration, psychopharmacology and medical emergency policy and procedures.10. Provide professional and personal development opportunities to participant and RHD Salt Passaic Team through the coordination and facilitation of medical and health related trainings, lunch & learns and one-on-one consultations, i.e., physical activity, sleep & rest, stress management, nutrition & healthy eating, diabetes, smoking, obesity, hypertension, annual health screening, etc.11. Maintain on-going communication with participant and RHD Salt Passaic Team and perform chart audits to ensure that staff documentation is accurately reflecting the progress towards identified medical health recovery goals and all appointments are being scheduled, attended, and follow-up is provided as per provider’s orders/requests, lab work, referral, specialty appointments, etc.12. Assist participant with establishing and maintaining relationships with medical vendors, including, supplies and transportation providers.13. Ensure the timely ordering of medical supplies, equipment and repairs, as needed.14. Attend RHD Salt Passaic Team meetings, interdisciplinary meetings, clinical conferences, stake holder meetings, and trainings.15. Assist in developing and maintaining a holistic, “participant-centered”, recovery focused milieu and process.16. Provide 24/7 on-call coverage for emergency coverage and consultation.*Required Experience*2 years experience*Required Education*Possess Licensed Registered Nurse (RN) with current license plus minimal 2 years of experience (co-occurring experience a plus).*Required Qualifications*1. Must consent to a NJ Criminal Background Check and a DHS Registry of Criminal DD Offenders Check.2. Possess a NJ valid driver’s license.3.
