Our post-acute, long term care facilities are hiring LPN’s / LVN’s with full time scheduling, competitive pay starting at $40 / hr (current crisis incentive comp rate), and compelling shift differentials!

Education requirements Must be a graduate of an approved school for practical or vocational nursing.

Experience requirements Experience is preferred in rehabilitative or geriatric nursing.

Qualifications Must be currently licensed as a LPN / LVN by the State Board of Nursing.

Must have a current CPR Certification.

Responsibilities The duties and responsibilities of the LPN include managing the day to day healthcare operations of the Community to ensure residents’ healthcare needs are met.

The LPN will ensure all residents are treated with respect and dignity as well as managing the quality of their care as their healthcare needs change.Assists in the maintenance of resident charts and review of documentation performed by care giving staffAs allowed by the Nurse Practice Act, assists in assessing the health, functional, and psycho social status of residentsEvaluates the effectiveness and maximizes the resident’s opportunity to remain in their environmentEnsures and conducts Personal Service Plan reviews and updates, according to the facilities and state requirements, to ensure residents’ service needs are metImplements timely problem solvingImplements and maintains the facilities operating standards, systems and processes.

Monitors and participates in Quality Improvement and Risk Management programs at Safety Committee Meetings, Collaborative Care Meetings and Family Care ConferencesAssists in the scheduling and follow through of physician visits including, but not limited to, dental appointments, eye appointments, therapy, and hospice.

Ensures communication, as appropriate, on the results of these visitsShares on-call responsibilities/shifts as requiredOther duties as assigned by management Work hours Various Shifts are available with shift differentials!
