Supervisor Community Based Health Managers – RN or LSW required


This is a Contract position in Media, NJ posted June 15, 2020.

Job Summary:

Responsible for oversight of Community Based Health Management team  activities including partnering with the VMG physicians and staff in coordinating patient care with the underlying objective of enhancing quality and cost effective care.  Acts as a resource and patient advocate to facilitate a safe transition across the continuum. Works in collaboration with Administration, VMG Physician Leadership to establish measurable standards and best outcomes for customers.  Ensures continued development of the program and identification of opportunities for improvement.

Position Responsibilities:

Care Coordination – Managing an abbreviated assignment, candidate will ensure appropriate departmental operations for care coordination through psychosocial assessment, multidisciplinary rounds and patient advocacy.  Communicates and educates staff and appropriate healthcare team members on the plan of care and post-acute transition options supporting patient freedom of choice. 

Supports the department Director in the daily operational oversight includes hiring, staff schedules, payroll, daily task assignments and Huddle coordination. Incorporated into daily tasks is addressing staff issues via coaching/counseling, mentoring, recognition and Best People Review.

Documentation – Provides ongoing review of open and closed medical records to ensure Quality and Compliance with all applicable government and commercial agencies for reimbursement.  Facilitates development of department targets and monitors to meet goals.  Identifies opportunities for improvement and communicates appropriately. 

Participates in organizational improvement activities, including patient satisfaction teams, six sigma teams, departmental/divisional teams, and committee participation.

Position Qualifications required:

Required Experience:

Required: Must be RN

Preferred: 3 years recent clinical nursing (RN) experience  Excellent verbal and written communication skills, problem solving, critical thinking; basic knowledge of understanding evidenced based data, and setting benchmarks and targets.  Basic understanding of Medicare, Medicaid and managed care. 

Required Education:

Graduate of an accredited School of Nursing, minimum BSN required.

Training / Certification / Licensures:

Must be RN in NJ.
